Four categories
Ohio uses category 1 non-excepted interstate, 2 excepted interstate, 3 non-excepted intrastate, and 4 excepted intrastate. Non-excepted drivers must maintain the medical qualification required for their category.
Electronic submission
For examinations on or after June 23, 2025, the certified medical examiner reports the result to FMCSA and FMCSA transmits it electronically to Ohio BMV. Ohio no longer relies on the driver delivering a new paper medical certificate for this workflow.
Timing and status
A certificate already recorded remains effective until expiration, but required medical status or variance documentation must not lapse. A prohibited or not-certified status can cause loss or downgrade of commercial privileges.
Ohio options
Self-certification may be updated through approved online, email, mail, or in-person channels; an in-person deputy-registrar transaction carries the listed service fee. An eligible intrastate driver may separately seek a PUCO provisional medical certification and must carry the waiver with the CDL.
