New Jersey CDL New Jersey Medical Self-Certification and Electronic Status

Every New Jersey CLP or CDL holder selects one category: non-excepted interstate, excepted interstate, non-excepted intrastate, or excepted intrastate. Category 1 or 3 normally requires medical qualification; drivers under 21 select category 3 or 4 because their commercial operation is intrastate only.

Medical examiners electronically submit examination results to the FMCSA National Registry, which transmits certification information to MVC. New Jersey no longer uses a paper MCSA-5876 as proof for this workflow. The driver remains responsible for ensuring the official record is current before operating.

The CDL Holder Self-Certification form is required for an initial CLP, an out-of-state CDL transfer, and CDL renewal, and it must be updated when the driver's commerce changes. A non-expired electronic medical certification must be present when the selected category or endorsement requires it.

New Jersey requires every P or P/S endorsement holder to maintain an electronic medical examiner certification regardless of whether the driver otherwise selects excepted interstate or excepted intrastate commerce. The passenger rule is therefore stricter than the general category 2 or 4 medical exception.

MVC requires a current medical status for covered operation and renewal. An expired or missing required certification can place the commercial privilege out of compliance and lead to downgrade or denial. A new exam is useful only after the electronic result reaches the National Registry and state record.
