Four categories
North Carolina uses interstate non-excepted, interstate excepted, intrastate non-excepted, and intrastate excepted categories. The two non-excepted categories require certified medical status; mixed interstate and intrastate operation is classified as interstate.
Electronic reporting
Beginning June 28, 2025, the medical examiner transmits examination results electronically through FMCSA's National Registry II system and NCDMV posts them to the driver record. A paper Medical Examiner Certificate is no longer accepted as proof by the driver or carrier.
Expiration and notices
A driver must renew the examination and any required variance before expiration. The current NCDMV page says an affected driver has 30 days to respond to a not-certified notice before North Carolina driving privileges are canceled.
Variance and record status
FMCSA transmits federal variances to NCDMV. North Carolina may separately require a medical report or state medical-review action when a condition raises fitness questions. A medical variance appears through restriction V.
