Four operating categories
Every CDL or CLP applicant and holder self-certifies to one category: non-excepted interstate, non-excepted intrastate, excepted interstate, or excepted intrastate. Non-excepted interstate and non-excepted intrastate drivers must maintain an unexpired qualified Medical Examiner's Certificate in the registry. Mixed excepted and non-excepted work is certified as non-excepted.
Electronic transmission
Beginning April 21, 2025, Missouri receives qualified MEC information electronically from the National Registry. The old online medical-certificate upload route was removed. The certified examiner posts the result, the registry transmits it, and Missouri posts it to the commercial record when the credential and self-certification status permit the match.
When manual action remains
Drivers in excepted categories do not automatically receive a posted MEC merely because an examination occurred. A change to a non-excepted category is completed in person so Missouri can update self-certification and query the registry. State or federal SPE, waiver, or exemption documents may still have to be presented and carried as directed.
Carry rule and variance
As of June 23, 2025, a driver whose current MEC information is posted to the record under the applicable federal rules no longer needs to carry the physical MEC copy while operating. This does not eliminate a requirement to maintain and carry an applicable medical variance or waiver document.
Expiration and downgrade
Missouri notifies a driver whose medical certification is no longer current. If updated MEC and variance information are not posted within 60 days after expiration, the state downgrades the license to remove CDL privileges. A new MEC may restore status without retesting in some cases, but voluntary downgrade or extended credential expiration can change the recovery path.

Separate the nationally shared operating rule from the Missouri transaction rule. On an exam or at an office, apply the exact class, endorsement, restriction, document, fee, date, medical status, vehicle, and testing condition stated in the current Missouri source.
