Alabama CDL Alabama Medical Self-Certification and Electronic DOT Record

Every Alabama CDL or CLP holder identifies the actual operation as non-excepted interstate, excepted interstate, non-excepted intrastate, or excepted intrastate. A new self-certification is needed when the type of operation changes, not merely because time passes.

Beginning June 23, 2025, ALEA accepts DOT medical certification only through electronic submission from an authorized medical examiner through FMCSA. Paper cards and driver-uploaded medical cards are no longer accepted for updating the medical result.

ALEA receives the medical result electronically and attaches it to the driver record. The holder should use the Alabama online account to verify both self-certification and medical status before applying, renewing, upgrading, adding an endorsement, transferring, or operating.

If required self-certification, medical certification, or variance information is not received within 45 days after ALEA's notice, all CDL privileges are removed from the license. The notice interval is not permission to continue commercial driving while medically unqualified.

When correct required medical information reaches the record, ALEA states that medical downgrade privileges can be reinstated without a reinstatement charge. If the Alabama CDL has been expired more than three years, re-examination is required instead of simple restoration.
